Do I need to install Fractal Mod myself?

Only if another mod lists it as a required dependency. Fractal is a library mod that doesn't add content on its own, so you'll only need it if something else depends on it.

Why do some mods have subgroups in creative inventory and others don't?

The mod developer must have built their mod to use Fractal's subgroup feature. If a mod doesn't use Fractal, its items will appear in a single tab without subgroups, even if Fractal is installed.

How do I switch between subgroups?

Open the creative inventory and look for subgroup tabs on the left side of the screen. Click the tabs to switch between different subgroups within the same mod's category.

Can a mod have more than twelve subgroups?

No, Fractal allows up to twelve subgroups per mod tab. If a developer needs more organization, they would need to use a different approach.
