How do I get the charm slot to show up in my inventory?

Do I need to hold the totem in my hand for it to work with this mod?

Can I put the totem in my offhand instead of the charm slot?
Yes, vanilla Minecraft already lets you hold a totem in your offhand and it will still save you. The charm slot is simply an extra dedicated space so your offhand stays free for other items.
Why isn't my charm slot totem saving me when I die?
