Cake variants are assigned randomly when you place a cake block in the world. Each time you place a cake, it will display one of the 26 unique textures from the pack.

No, the variant is randomly selected when placed. If you want a specific cake design, you'll need to place cakes repeatedly until you get the one you want.
Do cake variants have different effects or behaviors?
No, all variants are purely cosmetic. They look different but function identically—eating them restores the same hunger and saturation as regular cake.
