How to Fix Player Heads That Won't Generate or Look Wrong
Command Fails or Returns Nothing
If /jph does nothing, the most common cause is a name that does not resolve to a real account. The mod looks up the player's profile and skin from Mojang's servers, so a typo or a name that has never existed will fail. Re-type the name carefully and try again.
A second cause is rate limiting. The lookup service has request limits, so if you fire off many commands in a row, some may be rejected.
Head Shows a Default or Wrong Skin
If the head appears but has a blank or default texture, the profile lookup likely succeeded for the account but the skin data was not available at that moment. Try generating the head again later. Heads generated from a player's name at a time when their skin was different will keep the older texture, since the head stores the texture data it was created with.
If you are on a server, confirm the server has outbound access to the profile service — a blocked connection will cause lookups to fail even though the command syntax is correct.
