How the Totem Activates
The Totem of Undying automatically triggers when you receive damage that would kill you. It consumes itself in the process, restoring your health and giving you temporary protection effects. The totem must be in your off-hand slot to work—it won't activate if it's only in your main inventory.
What the Totem Does and Doesn't Protect Against
The totem saves you from most types of fatal damage, including fall damage, mob attacks, and environmental hazards. However, it has specific limitations: it cannot protect you from the void, and certain instant-kill effects may bypass it depending on your Minecraft version. Always have a backup totem ready since each one is consumed after activation.
Plan Your Inventory Around Totems
Since totems take up an off-hand slot, you won't be able to use a shield or other off-hand items simultaneously. Plan your loadout accordingly—choose between shield defense or totem protection based on your situation. For boss fights where you expect heavy damage, the totem is usually the better choice.
