Plant on soul sand as usual
Nether wart grows on soul sand, not farmland. Place your nether wart as you normally would and let it mature. The mod treats nether wart the same way it treats overworld crops, so the replanting logic applies here too.
Break mature wart while standing on the soul sand
When you break a fully grown nether wart, the mod uses one of the dropped nether wart items to replant it directly back onto the same soul sand block. You do not need to hold nether wart in your hand — the drop itself is consumed for the replant. Walk along your rows breaking each mature plant and the patch will stay planted.
Suppress replanting when clearing
If you want to harvest everything and leave the soul sand empty (for example, to relocate your wart farm), hold sneak while breaking. Just like with overworld crops, sneaking prevents the automatic replant so the soul sand is left bare.
Practical tips
Nether wart does not need light or water, so you can build dense farms. Because the mod replants from drops, a fully mature plant always has something to replant with. Avoid breaking immature wart if you want the block to stay planted, since an immature plant may not drop a wart to replant from.
