First Checks
If no bars appear at all, confirm the mod is actually enabled for the instance you launched — it's easy to have it installed in one profile and launch another. Next, check the max-distance setting: if it's set very low, entities beyond a few blocks won't show bars, which can look like the mod is broken. Raise it and walk up to a mob to test.
Also verify you haven't accidentally switched the display to whitelist mode with an empty list, or blacklisted the entity type you're testing with. Reset the filter to defaults to rule this out.
Conflicts and Rendering
If bars show for some entities but not others, it's almost always the filter or a per-entity toggle rather than a bug. If bars are missing entirely and settings look correct, suspect a conflict with another HUD or nameplate mod that also draws above entities. Temporarily disable other overlay mods and re-test.
Finally, check that your graphics settings aren't interfering — some rendering options and shader packs can hide or obscure overlay elements. Try without shaders to isolate the cause. If bars appear only when close but vanish at range, that's the distance setting, not a fault. After changing any config value, leave and re-enter the area so entities are re-scanned.
